A&Z Health Services, Inc. is required by law to protect the privacy of your protected health information (PHI), to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect. - For treatment — to provide, coordinate, and manage your home health care among your nurses, therapists, aides, and physicians.
- For payment — to bill and obtain payment from Medicare, Medicaid, insurers, or you.
- For health care operations — for quality, training, licensing, and administrative functions.
- As required by law — including public health, safety, and other legally required disclosures.
