Medical social services help patients and families who feel overwhelmed by the non-medical side of care — the planning, paperwork, and hard decisions that come with illness.

Coverage & payment

Medical social services are provided as part of your home health plan of care when you're also receiving skilled nursing or therapy, and are covered by Medicare for eligible patients. We'll help you understand what applies — just call.

From your first call to the first visit at home.

Call us or request care

Reach us by phone or send a quick request. We'll gather the basics and help confirm coverage — usually the same business day.

A nurse visits and builds the plan

A registered nurse visits the home for a full assessment, reviews your physician's orders, and builds a care plan around real needs.

Care begins at home

Your assigned clinician begins visits on the agreed schedule, and we keep you and your doctor informed along the way.

For physicians & discharge planners

We make referrals easy: same-day acknowledgment, start of care within 24–48 hours of receiving orders, and written clinical updates after every visit. See our referral page or call (813) 400-0324.
